What you’ll learn

LO1 — Explain pressure injury pathophysiology and identify intrinsic, extrinsic and device-related risk factors.
LO2 — Perform and document a structured skin and pressure-injury risk assessment, including correct use of the six Braden Scale subscales.
LO3 — Differentiate Stage 1, Stage 2, Stage 3, Stage 4, unstageable pressure injury and deep tissue pressure injury, and recognize injuries that should not be staged using the pressure-injury system.
LO4 — Develop an individualized prevention plan addressing pressure redistribution, repositioning, moisture, nutrition, friction/shear, mobility and medical devices.
LO5 — Recognize deterioration or newly identified pressure injury, initiate immediate protective actions, document accurately, educate the patient/family and escalate according to local policy.

About this module

MOD-011 is module 1 of 5 in 1.3 Fundamentals of Care & Nutrition, inside Domain 1: Nursing Fundamentals & Clinical Essentials.

Adult pressure injury risk assessment, Braden Scale use, comprehensive skin assessment, NPIAP staging principles, individualized prevention, medical-device-related pressure injury, documentation, escalation and GCC practice. Local wound-care policy, approved risk thresholds, repositioning schedules, support-surface algorithms and specialist referral criteria always take precedence.

The course runs the full NMS competency journey: a diagnostic that maps your starting point, 28 illustrated lessons built from the approved course pack, 2 branching clinical scenarios, a digital simulation, quick-reference and flashcard revision, and a 41-question summative assessment at an 80% pass mark.
